Caregiver Proxy Forms

Product Description

The Caregiver Proxy Form can be used to gather valuable caregiver perspectives and their perception of the child’s strengths across the CARE Model. The Caregiver Proxy Form, provided to caregivers of children of all ages (5-18), includes free response questions, a caregiver rating of their child’s overall communication competence, 11-point scale questions, and true-false questions. Written instructions for caregivers are provided in the Caregiver Proxy Form for independent self-administration of all subtests.

Teacher Proxy Forms

Product Description

The Teacher Proxy Form can be used to gain insights into the child’s strengths across the CARE Model as observed in the classroom. Each form includes free response questions, a teacher rating of the child’s overall communication competence, and 11-point scale questions. Written instructions for teachers are provided in the Teacher Proxy Form for independent self-administration of all subtests, and scoring is embedded.
